In order to plan for support services, it is necessary to gather information and determine the needs of individuals. Each individual must go through Mental health pre-screening Checklist provided by Division of Developmental Disabilities (DDD) . This Assessment Tool assesses individual competencies and determines relative need for service. The Assessment Tool is designed to assess individuals with developmental disabilities in the areas of cognition, communication, self-care and mobility.
Once a level of need is determined, this information is used to develop a budget. Family members of individuals with developmental disabilities with the lowest competencies spend the most support time and have the highest need for services.
Services are based on assisting individuals and families to create outcomes through a Person-Centered thinking model called the Person-Centered Planning Tool (PCPT). This is a strength-based tool used to guide the person centered planning process and to assist in the development of an individual’s service plan (ISP). This tool helps individuals and their families to go beyond the identification of available services and allows them to focus on what is important to them.
