Job Description: Support Coordinator

  

Responsibilities

· 

· Actively participating in team meetings.

· · Writing the PCPT as a result of the person-centered planning process and by incorporating previously developed person-centered planning documents (from schools, other States, Family members, etc.)

· Writing the ISP in clean and understandable language based on the consensus reached during the team meeting.

· ISP are available in all settings where the individual receives services.

· Ensuring that all data is entered into the iRecord;

· Monitoring and reviewing the ISP.

· Annual/Quarterly face-to-face visits (3-4x a year);

· Develop and maintain knowledge of client services through the Monthly tool (MMT)

· Ensuring the Participant’s budget is maintained

· Identify a Service Provider for the participant based on the participant’s needs

· Handle participant inquiries, document, and update participant information in iRecord.

· Update any new benefit information and secondary insurance information as needed.

· Frequent contact with individuals within the department and outside the organization

· Ensuring that all documents that need to be retained are adequately stored, scanned, and logged

· Other relevant responsibilities are outlined in Section 6.3 of this manual.


Qualifications

·   

· A bachelor’s degree with a preference in Social Work or a related human services field

 Strong writing and communication skills, adaptability, commitment to clients, and attention to detail

· A minimum of one year of experience working with adults with intellectual disabilities

· A valid driver’s license and a vehicle

· Must be exceptionally organized and willing to learn. 

· Bilingual preferred


Benefits

· This position is a REMOTE Job with a 4-location visit in a year

· Pay varies upon experience

· DDD training and in-house training is $18/hr. 

· Flexible schedule

· File 1099.
