The Support Coordinator will:
· Identify and Coordinate meeting with client based on iRecord data
· Actively participating in team meetings
· Gather accurate data and schedule appointments
· Develop and Maintain Individual Service Plan (ISP) & Person Centered Plan Tools (PCPT)
· Monitor and Review Data
· Identify service provider for client, based on clients needs
· Attend periodic meetings
Requirement:
Associate Degree or higher
1-2 years experience, working with individuals with developmental disabilities
Ability to read, write and great listener
Ability to coordinate tasks and work with others
Proficient in Microsoft Word, Excel, Search engine, Data entry and Phone usage
Ability to drive or take Public transportation
