It is the process by which the state helps a family with a child with developmental disability get most of the resources needed for daily support through a Support Coordinator.
The Support Coordinator will complete the following tasks:
1. Finds you benefits — SSI, food stamps, Medicaid, housing help
2. Builds your plan — called a Service Plan or ISP. Lists what you need: therapy, day program, job coach, housing, personal care
3. Finds providers — in your County who takes your Medicaid, and has openings
4. Manages budget — if you get a DDD/Medicaid budget, they help you spend it right
5. Checks in monthly — makes sure you're not losing services, helps with appeals
Misconceptions:
SC will help you get help without your involvement: please note that an SC will guide and support you with the help you need but will not follow you to the location; DSPS and SB can be hired to do so.

Support Coordinator (SC): works with participants or the participant's guardian and service providers to advocate for, identify, develop, coordinate, and access services on behalf of the participant.
Support Coordination empowers individuals with disabilities and their families to stay in control of day-to-day activities by ensuring participants have a home, work-life balance, and the ability to function in their community.

New Jersey Comprehensive Assessment Tool
To plan support services, you must gather information and determine the individual's needs. The NJCAT assesses individuals' competencies and determines relative need for services. The assessment tool is designed to assess individuals with developmental disabilities in cognition, communication, self-care, and mobility.
PCPT/ISP
Services are based on assisting individuals and families to create outcomes through a Person-Centered thinking model called the Person-Centered Planning Tool (PCPT). This strength-based tool guides the person-centered planning process and helps develop an individual’s service plan (ISP). This tool helps individuals and their families go beyond identifying available services and focus on what matters to them.
Individualized Budgets
The NJCATs determine an individual budget. Once the NJCATs determine a level of need, they use that information to develop a budget. On the norm, an individual with the lowest competencies spends the most support time and has the highest need for services.
